Most frequently, the frontal areas are damaged, however, the destruction can spread onto other regions and grey matter structures. The disease begins inconspicuously and progresses slowly, the symptoms increase in several years.  With age or if the treatment is absent, the pathological process actively disorders the neurons and finally the dementia onsets. Cortical atrophy of the brain mostly develops in people after 60, but in some cases the destructive processes are observed at earlier age due to genetic predisposition.

Two hemispheres are damaged by cortical atrophy in Alzheimer’s disease. The pronounced disease leads to full idiocy while small destructive foci do not significantly affect a human mind. The strength of clinical symptoms also depends on localization and severity of the damage of the subcortical structures or cortex. Moreover, the speed of progress and dissemination of the process mut be taken into account.
